Documentation Appraisal Workshop: Find Errors with Opportunities

The teaching objective of this day is to be able to define the types of federal auditing organizations, what triggers audits and discuss solutions to alleviate errors found during audits. Attendees will gain an understanding of Medicare`s national and local coverage determinations for common endocrine procedures and services such as HbA1c, Vitamin D, DXA, TSH etc. Determine the key components in an E/M visit using endocrine specific examples and explanations for 95 and 97 E/M guidelines nad discuss opportunities to provide clear and appropriate documentation using real life endocrine notes.

Physicians, Billers, Coders, Office Staff, Practice/ Office Administrators, Non-Physician Practitioners.

Learning Objectives

  • Define types of federal auditing organizations and briefly discuss what can trigger audits.     
  • Discuss common errors found in audits and provide solutions to alleviate the errors.
  • Define and discuss Medicare’s national and local coverage determinations on common endocrine procedures and services such as Diabetes Self-Management Training (DSMT), Continuous Glucose Monitoring (CGM), Vitamin D, HGBA1C, thyroid and lipid testing.
  • Define and discuss the types of evaluation and management codes reported by physicians based on location and provide the elements required for each level visit based on CPT® guidelines. 
  • Define and discuss the key components (history, exam and medical decision-making) in an E/M visit utilizing endocrine specific examples and explanations for 95 and 97 E/M guidelines. 
  • Define and discuss the key components (history, exam and medical decision-making) in an E/M visit utilizing endocrine specific examples and explanations for 95 and 97 E/M guidelines.      
  • This hands-on exercise will provide attendees the opportunity to discuss the necessity of clear and appropriate documentation using real life endocrine notes and auditing principles.
